Acknowledgement of Risk and Waiver Liability and Concussion Statement
Read this Completely before signing. Its Effect is to Release Grays Harbor College from any Liability resulting from your participation in the activities described below and to waive all claims for damages or losses against the college that may arise from such activities.
Activity: Grays Harbor College Youth Camps
The undersigned participant, exercising my own free choice to participate voluntarily in the activities mentioned earlier and promising to take due care during such participation, at this moment release and discharge, indemnify and hold harmless Grays Harbor College, and their members, officers, agents, employees, and any other persons and entities acting on their behalf, and the successors and assigns of any and all of the aforementioned persons and entities, against any and all claims, demands, and causes of action whatsoever, whether presently known or unknown, either in law or in equity, relating to injury, disability, death or other harm, to person or property or both, arising from my participation in and/or presence at the above-listed activities.
I acknowledge that I have been informed of the nature of the activities and that I am aware of the hazards and risks that may be associated with my participation in the activities mentioned earlier, including the risks of bodily injury, death or damage to property which may occur from known or unknown causes. I understand, accept, and assume all such hazards and risks, and waive all claims against the State of Washington, the State Board for Community and Technical Colleges, Grays Harbor College, and other persons as set forth above. I understand that I am solely responsible for any costs arising out of any bodily injury or property damage sustained through my participation in normal or unusual acts associated with the activities mentioned earlier.
Concussion Policy:
- Any athlete suspected of sustaining a concussion will be immediately removed from all athletic activities for the remainder of the day.
- The athlete may not return to participation until evaluated and cleared in writing by a licensed health care provider trained in concussion management.
- Parents/Guardians will be notified promptly if a concussion is suspected.
- All concussion management will follow Washington State Law (RCW 28A.600.190) and recognized national safety guidelines for youth sports.
I have read and fully understand this acknowledgment and consent, and I am signing it voluntarily. I now knowingly accept and choose to encounter all responsibilities and risks associated with my participation in this activity. I further understand that behavior inconsistent with the terms herein may result in dismissal from Grays Harbor College.
