Please be aware that an appeal for reinstatement of Financial Aid is intended to be a ONE TIME ONLY exception for a student who had an unexpected, extenuating circumstance leading to non-satisfactory academic progress. Complete this form and all parts to the best of your ability. Be aware that submitting a Suspension Appeal form does not guarantee approval and that incomplete appeals will be denied.

Appeals are generally acceptable for the following reasons:
- An unexpected medical emergency (attach documentation)
- A death in the immediate family (attach documentation)
- A sudden, unexpected loss of housing or transportation (if possible, attach documentation)
- Other unpredictable conditions/circumstances
Appeals can NOT be accepted for the following reasons:
- You didn't like your class/instructor
- You couldn't attend class due to work/childcare conflicts (unless there was an emergent situation that you couldn't plan for)
- You couldn't afford your textbooks/materials/bills/etc.

Your program of study information (degree or certificate & major) will be based on your declared program as listed with Enrollment Services. You may see what your declared program is by printing out a copy of your current class schedule. If you need to change your program, please submit a major change form to the Student Services Center.
Student Personal Statement
• Answer the following three questions. Please note this is your chance to explain your circumstances, so take the time to thoroughly answer each question. Remember the 5 W's: Who, What, When, Why, and Where. Please attach documentation of your circumstance at the bottom of this form if possible. We may contact you for more documentation if we require it.

Education Plan
Complete an education plan with your academic advisor that includes only classes that are required to complete your degree for each of your remaining quarters at GHC. This plan must be approved and signed by your academic advisor, and remember to keep a copy of your education plan for your reference. If you are unsure of who your academic advisor is, please log into the MyGHC Student Portal.

If your assigned academic advisor is unavailable such as during winter, spring, and summer breaks, stop by or call the Student Support Center, (360) 538-4099, to make an appointment with a counselor or advisor to help you create an approved and signed plan.

I certify that the information provided in this appeal is true and accurate. I also understand that the decision of the Appeal Committee is final, and I agree to comply with any restrictions stipulated by the Committee. If my appeal is approved I may be reinstated on a probationary status or placed on a Scholastic Expectation Plan (SEP) until I attain good standing or complete my program of study. Failure to complete my probationary quarter or follow the SEP will result in the termination of my financial aid eligibility without the option to appeal.
