Your program of study information (degree or certificate & major) will be based on your declared program as listed with Enrollment Services. You may see what your declared program is by printing out a copy of your current class schedule. If you need to change your program, please submit a major change form to the Welcome Center.
Answer the following three questions. Please note this is your chance to explain your circumstances, so take the time to thoroughly answer each question. Remember the 5 W's: Who, What, When, Why, and Where.
Complete an education plan with your academic advisor that includes only classes that are required to complete your degree for each of your remaining quarters at GHC. This plan must be approved and signed by your academic advisor, and remember to keep a copy of your education plan for your reference. If you are unsure of who your academic advisor is, please log into the MyGHC Student Portal.
If your assigned academic advisor is unavailable such as during winter, spring, and summer breaks, stop by or call the Student Support Center, (360) 538-4099, to make an appointment with a counselor or advisor to help you create an approved and signed plan.
If you have a current plan please provide it by selecting option 1 or 2. If you need to work with your advisor to create or revise your plan please select option 3.

I certify that the information provided in this appeal is true and accurate. I also understand that the decision of the Appeal Committee is final, and I agree to comply with any restrictions stipulated by the Committee. If my appeal is approved I may be reinstated on a probationary status or placed on a Scholastic Expectation Plan (SEP) until I attain good standing or complete my program of study. Failure to follow the SEP will result in the termination of my financial aid eligibility without the option to appeal.
